IMMIGRATION PHYSICALS

Dr. Aazrum Syed is a certified civil surgeon designated by the United States Citizenship and Immigration Services (USCIS). The examination includes the following:

· Physical exam by our civil surgeon.

· TB testing (age 2 years and up)

· Blood tests for Syphilis & Urine and test for Gonorrhea (age 15 years and up)

o Discussing and recommending vaccination requirements for the completion of your I-693 form.

§ Sealing the completed I-693 form in secure envelope and providing full copy records for the patient.

Vaccination Requirements:

By law, for the immigration physical exam the required vaccines include (you need at least one of each):

1. Mumps, measles, rubella (MMR)

2. Tetanus and diphtheria toxoids, Td or Tdap

3. Hepatitis B

4. Varicella

5. Influenza – only if in season (Sept 1st – March 31st)

6. COVID – 19 Vaccine(s)

Without proof of vaccinations, we cannot complete your forms. If you are certain that you have a current vaccine status but do not have the paperwork proof, we can run the laboratory testing for immunity at an additional cost per test. Keep in mind if the labs show no immunity, you are still required to get new vaccines.

Vaccines can also be administered in our office for an additional fee:

Td – Flu –

These are the only vaccinations we carry in office. Other vaccinations may be required:

Tetanus Diphtheria titer – additional MMR tier – additional

Varicella titer – additional Hepatitis B –

Additional costs may be incurred if labs come back positive. If QuantiFERON Tuberculosis test is positive you will be required to get a chest X – ray. The price of the physical does not include the chest X -ray, however, you may be able to use your private health insurance to pay for the X –

ray. If Syphilis screen is positive then you will need additional labs for confirmation at an additional cost.

When you are coming for your immigration exam, please remember to bring the following documents:

· Passport or other government issued photo ID (Must be current)

Dr. Syed is not responsible for determining whether an applicant is available for adjustment in status. That determination is made by a USCIS officer after reviewing records, including the report for the medical examination.
